What you will be doing :
Conduct thorough assessments of patients' health conditions in their home environments.
Develop personalized care plans and collaborate with multidisciplinary teams to deliver optimal patient care.
Administer medications and treatments as prescribed by the physician.
Provide education and support to patients and their families on disease prevention and health maintenance.
Monitor patient progress and modify care plans as necessary to achieve health goals.
Maintain accurate, detailed reports and records in accordance with regulatory requirements.
Ensure compliance with all healthcare regulations and quality standards.
Experience you will need :
Current Registered Nurse (RN) license in the state of Minnesota.
Minimum of 1-2 years of experience in a home health setting.
Strong assessment and clinical skills with a focus on patient-centered care.
Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare teams.
Ability to work independently and manage time effectively in a home health environment.
