Essential responsibilities:
The Extensivist Provider is a mid-level provider who works independently to conduct Annual Medicare Wellness Visits. Primary duties include health risk assessments, depression screening, medical and social history, risk factors and health conditions management, preventive screening schedule, patient education and referral management and advanced care planning.
A Medical Wellness Provider follows evidence based practices and standards of care for the Medicare Wellness patient population based on Centers for Medicare and Medicaid guidelines. The Provider will need to collaborate with patients, caregivers, and the patient’s primary care provider to develop a comprehensive plan of care. In doing so, they will identify care gaps that are due to be closed and work with patients to get procedures scheduled for required preventive screenings as needed. They may also identify educational needs of the patient/family and provide instruction and community resources /referrals to meet patient healthcare goals. Along with a general knowledge of Geriatric, Adult Medicine and Family Medicine, the Provider will demonstrate sound clinical judgment and critical thinking to identify situations when a physician consultation is needed during or promptly following a Medicare Wellness Visit to address acute medical needs.
Minimum qualifications:
Medical Wellness Providers must be a board certified Nurse Practitioner (NP) or Clinical Nurse Specialist (CNS) with clinical experience (2 to 4 years preferred.) Must have an active license in the State of Texas.
