What are the responsibilities and job description for the Program Manager FBS DD Waiver position at External Opportunities?
I. Essential Job Duties and Responsibilities:
Department Functions: Functions as the team leader in treatment coordination for individuals with intellectual, developmental and cognitive disabilities (IDCD) receiving ARCA Community Services, Family Based Services. ISP/IDT: Acts as Treatment Coordinator for all clients. Waiver Programs: Applies for DD Waiver, Medically Fragile and TBI Waiver programs as deemed appropriate for all individuals on his/her caseload. Intake: Program Managers participate in intake duties in FBS. This includes, but is not limited: to answering requests for information, sending out application packets, securing funding, either private contract of DD Waiver, and requesting appropriate information. Health and Safety: Works with the ARCA Safety Coordinator to assess the safety of the home and licensure requirements. Face to Face Contact: Makes regular face to face contact with the individual at least monthly. Recordkeeping: Ensures the individuals' assigned to them have the proper documentation within the file contents. Obtains information from teams and conduct assessments in order to meet content requirements. Filing: Keeps confidential files current and writes comprehensive Social Histories, Monthly Reports, Contact Notes, IDT/ISP Meeting Minutes and other required documentation. Professionalism: Must be a positive professional agency representative and confident advocate of individual receiving services. Staff Supervision: Supervises and annually evaluates the Family Provider on his/her caseload. Training: Completes required orientation training and on-going in-service training in accordance with ARCA policy on training. Abuse/Neglect/Emergency Situations: Treats individuals served with dignity and respect at all times. As required by law, reports all suspected instances of abuse, neglect, exploitation, environmental hazard, suspicious injury and/or injury of unknown origin to the appropriate authorities in accordance with ARCA policies and procedures. HIPAA Compliance & Confidentiality: Has full access to Protected Health Information (PHI) to carry out job responsibilities in compliance with HIPAA regulations and ARCA policies and procedures on confidentiality.
II. Typical Physical, Environmental & Mental Demands:
The majority of job duties of this position can be done at a desk, using the telephone, basic office equipment and computer. Ability to access files is essential, as is the ability to drive to meetings, doctor appointments, day services, Social Security Administration (SSA) and to transport individuals/families when necessary. Requires mobility to observe, supervise and assist individuals to carry out their daily activities. May require the ability to provide physical guidance to individuals who require assistance with walking, personal care or other physical activities. May require the ability to lift and transfer an individual in or out of a wheelchair.
Most work time is spent in the ARCA main office, homes, and day service providers for meetings. There will also be travel to doctor's offices, family homes, SSA other agencies within the city. This position includes staff support and direct service with individuals in their homes and/or within the community.
The stamina to complete a high volume of paperwork within deadlines is crucial. Ability to communicate is essential, as is the ability to remain calm and professional in stressful situations. Requires the ability to communicate effectively with individuals that may have limited speech skills or who are non-verbal. Requires the ability to develop goals, objectives, and daily routines.
III. Qualifications/Performance Requirements:
Must have basic knowledge regarding individuals with IDCD. Must have knowledge of IDEA (Individuals with Disabilities Education Act) regulations. Must have knowledge of and ability to access community resources. Must have knowledge of various funding sources. Must have knowledge of and experience working with families. Must be able to demonstrate skills in oral and written communication, organization and time management. Must possess the ability to mediate in situations of conflict. Must demonstrate the ability to advocate for individuals receiving services. Must demonstrate the ability to complete a high volume of paperwork by deadline. Must be able to testify in court. Interpersonal Skills: Ability to communicate effectively (both verbally and in writing) and maintain positive working relationships with individuals receiving services, staff, families, service providers and other entities as necessary. Must be able to work with a diverse group of people of varying cultural and educational backgrounds. Computer/Technology: Must possess current applicable computer skills.
Requirements: Driving: Current, valid New Mexico driver's license, a good driving record and the use of an insured, enclosed vehicle
Education/Experience/Credentials: Minimum of one (1) year’s experience working with individuals with IDCD preferred
Status: Full-Time, Non-Exempt
Hourly Pay: $24.50 No Degree, $25.50 Bachelor's Degree, $26.50 Master's Degree
Schedule: Monday thru Friday or other to meet program needs. Provides on-call coverage several weeks a year.
