What are the responsibilities and job description for the Referral Coordinator position at Eye Associates Of New Mexico?
POSITION SUMMARY:
This position supports the patient referral service for the Doctor2Doctor department. Utilizing EPM, EHR, Document Management, Phreesia and other support systems, the Referral Services Representative will input, generate, quality control, edit, track and actively monitor the various stages of treatment and associated disposition within the system. This includes the production/review of reports, communication with patients, and external referral sources. This position works closely with Clinical Referral Coordinator as needed to determine appropriate scheduling of external referral patients. Communication will be in both verbal and written form.
ESSENTIAL FUNCTIONS/RESPONSIBILITIES:
- Maintains and utilizes the consult/referral patient tracking database in association with information maintained within the EPM and EHR systems.
- Maintains and utilizes the referral tracking program within Phreesia.
- Communicates with external referral sources regarding the disposition of referred patients.
- Generates reports, reviews patient status and maintains current contact information for referral sources.
- Works closely with Clinical Referral Coordinator to determine appropriate referral scheduling (timeframe, specialty, etc.)
- Serves as the first point of contact for external referral sources to contact EANM regarding the care of their patients.
- Contacts patients to schedule their referral appointments.
- Coordinates the timely transfer of consult/care related calls to EANM doctors.
- Notifies appropriate clinic staff of appointments and information necessary to prepare for patient visits.
- Initiates and completes telephone interactions with patients, doctors and others in a professional, courteous and efficient manner.
- Interacts with Patient Accounting staff regarding insurance eligibility or other insurance needs related to their initial consultation (for incoming and outgoing referrals.)
- Assists with scanning and/or indexing of patient information in to EHR.
- Coordinates outgoing referrals on behalf of EANM providers to external referral sources.
- Serves as an ongoing training resource for department leaders.
- Assists with creation of new tracking systems, as requested by the Supervisor and/or Team Lead.
- Oversees workflow and department issues in the absence of the Supervisor and/or Team Lead, as requested.
- Communicates directly with doctors on issues, as requested by the Supervisor and/or Team Lead.
- Participates in both internal and external committees, meetings and/or events as requested by the Referral Services Supervisor and/or Director of Patient Access.
- Other duties as assigned.
SKILLS/QUALIFICATIONS:
- High school education or equivalent required.
- Microsoft Office Experience preferred
- Exceptional Customer Service Skills and high level of professionalism required.
- Must have effective verbal and written communication skills.
- Medical terminology and medical office experience preferred.
- Must be self-motivated and able to function independently in a multi-tasking environment, as well as a member of a team.
- Flexible, positive attitude.
- Must be willing to travel to EANM locations, meetings or events as needed
