ORGANIZATION OVERVIEW :
Face to Face Health and Counseling is a non-profit organization dedicated to advancing economic and health equity for youth ages 11-24. We do this work through a comprehensive approach that includes housing, mental health, medical, youth justice, education & employment, and community services. We take a strengths-based and trauma-informed approach to our work and center the needs of young people impacted by racism and other forms of oppression and marginalization. Founded in 1972, Face to Face serves more than 3,000 youth annually.
JOB SUMMARY :
This position provides intensive children's mental health case management services to children (up to 21) who meet criteria for the category of severely emotionally disturbed (SED) in Ramsey County. This position is full time at 37.5 hours a week.
RESPONSIBILITIES :
- Review diagnostic assessments of children with severe emotional disturbance to provide appropriate referral services for clients and families based on diagnosis and treatment needs
- Develop Individual Family Community Support Plans and Crisis Plans with child, family, professionals, and community, based on Diagnostic Assessment and functioning
- Assist the child and child's family in obtaining needed services by referral to and coordination with other agencies to support goal progress for assigned clients
- Assure continuity of care and reassess the effectiveness of services over time
- Work closely with child's school to support child in their individualized education plan
- Provide wrap-around support and care via coordination of meetings and contact with involved professionals such as health providers, child protection, school staff, and others who we have releases to speak with
- When clinically appropriate, assist in needs surrounding child's placement and discharge from residential treatment, foster care and other services
- Prepare reports and make recommendations to court as required
- Maintain paper formats of case records, maintain electronic charts in Apricot database, and include necessary documentation in County SSIS system
- Collaborate with other staff and county team members
- Maintain professional working relationships with clients, other agencies, and contacts
- Participate in clinical supervision, group consultation, and staff meetings
- Comply with the agency's written employment policies
- Model behavior consistent with, and supportive of, healthy adolescent development
- Provides all services guided by the 9 evidence-based principles for helping youth overcome homelessness
- Establish and maintain healthy and appropriate physical and emotional boundaries with youth and their families
- Maintain professional licensure, if applicable, and uphold the ethical guidelines of your licensing board
- Perform other duties as assigned.

Hours, Pay and Benefits : This is a full-time position at 37.5 hours / week. This position requires evening availability. The pay for this position is between $30-31 an hour year based on qualifications. Face to Face offers benefits which include health, dental, short-term disability, long-term disability, life insurance, 403b account as well as 23 days of PTO and 11 paid holidays.
We can provide Clinical Supervision towards licensure as : LMFT / LICSW / LPCC.
