Performance Objectives

·         Provide care coordination, care management, and/or care delivery to individuals with significant medical, behavioral, and social complexities that require intensive clinical support.

·         Complete required nursing reports

·         Provide complex care management and vital assessments

·         Develop health care plan

·         Ability to multitask is a MUST

·         Act quickly and responsibly in cases of emergency

·         Observe and report changes in client's condition

·         Be a team player who is energetic, enthusiastic and goal oriented

·         Strong attention to detail

Requirements/Benefits:

·         We prefer a Bachelor's Degree in Nursing, but you will need to have a minimum of an Associate's Degree in Nursing
