Job Title: Director of Rehabilitation Department: Rehabilitation

Reports to: CEO 

 

 

Job Summary:

 

The Director of Rehab will provide practical established Rehab. Therapy skills to residents following Fahrney-Keedy’s policies and procedures. They will evaluate, treat, and supervise the administration of therapy treatments and services to residents receiving rehabilitation services with a prescription from a physician, podiatrist, dentist or chiropractor. They will facilitate and provide strong verbal and written communication skills with residents, family members, visitors, staff, etc.

 

Supervisory Responsibilities: Supervises the entire Rehab team at Fahrney Keedy. This includes management of their performance, time, attendance, and compliance with state regulations, compliance with company policy/procedure and adherence to best practices within the rehabilitation field.

 

Duties/Responsibilities:

 

  • Provides for the screening, evaluation, and treatment, and prevention of chronic conditions, illnesses and/or musculoskeletal injuries according to your scope of practice to residents.
  • Provides ongoing training, auditing and completing appropriate documentation to ensure staff meet or exceed Fahrney-Keedy’s expectations.
  • Performs all assigned tasks in accordance with our established policies and procedures.
  • See that all referral sources are advised of the clinic action for the case referred.
  • Participates in development, coordination and implementation of resident care and discharge plans, including completing any discharge instructions for family.
  • Answers questions and concerns from resident family members.
  • Evaluates resident’s progress with therapists and recommend discharge from services as indicated.
  • Maintains work assignments, and/or work schedules in completing and performing assigned tasks.
  • Reviews scheduling related information with the team daily including, but not limited to - resident information/hair appts/wound list/appts/home assessments etc. to help the therapist appropriately treat their residents
  • Responsible for coordination of the rehab team schedule. Weekly updates should be provided to the team and PRN employees contacted in advance as necessary to ensure appropriate staffing levels.
  • Completion of PT/OT communication sheet to assist in mapping out scheduling and ensuring that supervised visits and other documentation are in compliance.
  • Check Projections & enter projections for new admissions so that treatment minutes can be transferred to the assignment board.
  • Complete PDPM worksheets for new admissions.
  • Ensure that PT/OT/ST have matching medical dx for Med A and all codes match.
  • Enter the PDPM worksheet if there is an IPA and then enter IPA into projections.
  • Complete Payers including obtaining authorization as needed and then verify and match to Nethealth before adding to active caseload.
  • Attend and come prepared to the following meetings:
    • Clinical meetings
    • Medicare Meeting
    • UR Meetings
    • Grand Rounds
    • Care Plan Meetings-
    • Director Meetings
    • Better Together/Optum/Other Miscellaneous meetings will be provided via an invite in the email system
  • Daily documentation, including all Txs/TENs/PNs/billing
  • Proper entry and documentation of all residents receiving therapy in SNF and ALU and coordination with VP of Finance.
  • Collaboration with students/universities as required and completion of all mandatory screening procedures and university/facility paperwork completion.
  • Monitors and updates patient list in NetHealth as needed.
  • Facilitates ordering supplies with the facility central supply clerk as needed for the rehab department including needs for residents, the rehab gym, and kitchen.
  • Submit transfer status change forms as needed to nursing.
  • Updates Bed Rail Screens/Logs as needed.
  • Completes Hand Hygiene audits monthly or as needed.
  • Completes feeding assistant screens quarterly or as needed.
  • Complete diet slip/forms as needed for changes.
  • Ensure that logs required to be kept up to date for state survey purposes are maintained, including but not limited to – refrigerator/freezer/hydrocollator temperature logs, AED checks.
  • Completion of home assessments as needed
  • Completion of IPAs as required/needed.
  • Maintains active caseload binder and keeps continuously current.
  • Preparation and completion of Medicare packets.
  • Completion of weekly chart audits.
  • CMS and Covinti audits as required/needed.
  • Monitors productivity for the rehab department and makes improvements as able and needed.
  • Completes monthly close out in collaboration with the finance department for billing purposes.
  • Reviews and responds to resident council concerns as needed.
  • Complete quarterly QA reports.
  • Maintains ACP requirements.
  • Tracks rehab employee licensure and updates in appropriate areas.
  • Attends marketing/company events and fundraisers as able and as appropriate.
  • Completes weekly clinical progress notes, daily treatment encounter notes, rehab. discharge programs for each resident via Optima.
  • Respond to emails timely, send emails to team members as appropriate as well as field emails to the team that are pertinent to their jobs/patient care. In general, promotes and maintains good communication within department as well as outside parties.
  • Determines and recommends rehab. services in accordance with resident care plan of physicians who are responsible for general direction and care of resident.
  • Treats residents with the most current techniques following appropriate physician orders and ethical standards to therapy treatments according to State Practice Act.
  • Cooperates with inter-departmental staff as well as other facility staff to ensure that therapy services can be adequately maintained to meet the needs of the residents.
  • Creates and maintains an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift.
  • Reports all complaints and grievances made by a resident immediately.
  • Ensure therapy staff adhere to the minimum weekly productivity standard.
  • Supports the department and rehab. staff and occasionally assumes these responsibilities in their absence according to your State Practice Act.
  • Supervises the PTA as applicable based upon State Practice Act guidelines.
  • Agrees not to disclose assigned user ID code and password for accessing resident/facility information and promptly reports suspected or known violations of such disclosure to the supervisor.
  • Agrees not to disclose resident’s protected health information and promptly reports suspected or known violations of such disclosure to the supervisor.
  • Performs inventory orders and ensures ordered supplies are within budget.
  • Reports any known or suspected unauthorized attempts to access the facility’s information system to the supervisor.
  • Ensure that departmental computer workstations left unattended are properly logged off or the password protected automatic screensaver activates within established facility policy guidelines.
  • Reports work-related injuries or illnesses and/or occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals to DON, HRD and/or CEO.
  • Keeps call buttons within easy reach of each
  • resident.
  • Immediately notifies the Director of Nursing of any resident leaving/missing from the facility.
  • Follows established safety precautions in the performance of all duties.
  • Keeps excess supplies and equipment off the floor. Stores in designated areas.
  • Cleans, disinfects, and returns all resident care equipment to its designated storage area after each use.
  • Monitors all hazardous conditions and equipment safety, reporting issues to maintenance or applicable department as necessary.
  • Reports all safety violations.
  • Reports any communicable or infectious disease to the Infection Control Coordinator.
  • Follows established isolation precautions and procedures.
  • Wears and/or uses safety equipment and supplies.
  • Safely uses only the equipment you have been trained to use.
  • Practices appropriate handwashing and use of hand sanitizer at all times.
     

Required Skills/Abilities:

• Attend continuing education as required by law for maintaining all applicable licensure.

• Participate in program development, in-service training and educational programs related to COTA licensure.

• Provide education to Rehab. Department following CEU event.

• Must be able to read, write, speak and understand the English language.

• Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.

• Must be a supportive team member, contribute to and be an example of teamwork and team concept.

• Must possess the ability to make independent decisions when circumstances warrant such action.

• Must possess the ability to deal tactfully with staff, residents, family members, visitors, government agencies/personnel, and the general public.

• Must possess the ability and willingness to work harmoniously with other staff.

• Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.

• Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.

• Must be able to relate information concerning a resident’s condition.

• Must not pose a direct threat to the health and safety of other individuals in the workplace.

Education and Experience:

• Graduate of an accredited Therapy Program.

• Licensure in applicable state.

• 5 years’ supervisory, management or program development experience in a clinical setting preferred.

Physical Requirements:

• Must be able to move intermittently throughout the workday.

• Must be able to speak and write the English language in an understandable manner.

• Must be able to cope with the mental and emotional stress of the position.

• Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.

• Must function independently and have flexibility, personal integrity, and the ability to work effectively with residents, staff, and support agencies.

• Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination.

• Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility.

• Must not have a weight-lifting restriction. Must be able to assist with transfer of a resident up to 300 lbs. and lift 100 lbs. Must be able to push, pull, move and/or lift to a minimum height of 5 feet and be able to push, pull, move, and/or carry a minimum distance of 50 feet.

• May be necessary to assist in the evacuation of residents during emergency situations.
