About the Position
Fair Haven Community Health Care is a dynamic and growing organization seeking a Care Access Specialist to join our team. In this role, you will work closely with our clinical teams to identify and address the social determinants of health that impact our patients' ability to adhere to treatment plans.
As a Care Access Specialist, you will develop and evaluate shared plans of care, link patients with social needs to community resources, and conduct home visits as needed. You will also work collaboratively with other organizations and care providers to ensure seamless care transitions.
Responsibilities
- Coordinate client intakes and non-clinical services at our shelter- and street-based services.
- Screen for social determinants of health at least yearly using a validated screening instrument on all clients.
- Liaise with other organizations and care providers to link clients to available services.
Qualifications
A bachelor's degree in a health-related field or equivalent is required, along with a valid CT driver's license and/or access to reliable transportation. Experience in care coordination, working with teams, and using an electronic health record is also necessary. Bilingual Spanish/English is required for this position.
