Job Summary
We are seeking a highly skilled Harm Reduction Service Coordinator to join our team at Fair Haven Community Health Care. In this role, you will work closely with our clinical teams to identify and address the social determinants of health that impact our patients' ability to adhere to treatment plans.
As a Harm Reduction Service Coordinator, you will develop and evaluate shared plans of care, link patients with social needs to community resources, and conduct home visits as needed. You will also work collaboratively with other organizations and care providers to ensure seamless care transitions.
Responsibilities
- Screen for social determinants of health at least yearly using a validated screening instrument on all clients.
- Liaise with other organizations and care providers to link clients to available services.
- Develop and evaluate shared plans of care.
- Link patients with social needs to community resources.
Qualifications
A bachelor's degree in a health-related field or equivalent is required, along with a valid CT driver's license and/or access to reliable transportation. Experience in care coordination, working with teams, and using an electronic health record is also necessary. Bilingual Spanish/English is required for this position.
