What are the responsibilities and job description for the Licensed Vocational Nurse position at Fair Oaks?
The Licensed Vocational Nurse, LVN, is responsible for providing direct patient care under the supervision of a Registered Nurse. The LVN is responsible for following the plan of care, providing treatments, and working with the clinical staff and other healthcare team members to meet patient care goals.
General Accountabilities
· Provides direct patient care under Vocational Nursing License scope
· Implements patient plan of care under the supervision of a Registered Nurse (RN)
· Evaluates the needs of patients, assists in developing care plan and supervises the care provided by Certified Nursing Aides (CNA)
· Provides accurate and timely documentation consistent with the care provided
· Implements and maintains established policies and procedures relative to skin care treatment and interprets findings to applicable parties such as physicians and patient family members as appropriate
· Participates in administrative duties such as completing medical forms, reports, evaluations, studies and charting
· Provides written and oral reports or recommendations to physicians or Director of Patient Care Services (DPCS) concerning the status and care of patients
· Monitors patients and reports adverse reactions to medication and treatment to applicable team members such as physicians or DPCS
· Administers medication and treatments such as wound treatments and dressing changes as ordered by physicians
· Utilizes equipment and supplies efficiently and effectively
· Assists in the admission and/or discharge of patients
· Assists in patient rehabilitation activities when required such as range of motion, ambulation assistance, and providing emotional support
· Provides preventative health care services such as making recommendations to patients and family members regarding nutrition, prevention techniques and other ways to improve the general health and psychosocial status of the patient
· Maintains confidentiality of all patient information including protected health information and reports known or suspected incidents of unauthorized disclosure of such information
· Reports any complaints or grievances made by patients regarding nursing or medical care to DPCS and maintains a written record of any resident’s complaints or grievances with notation of actions taken
· Reports and investigates all allegations of resident abuse and/or misappropriation of resident property
· Ensures that nursing staff honor the patient’s right to refuse treatment and verifies that such requests are in accordance with the facility’s policies governing advance directives.
· Gives direct physical and psychological nursing care in emergencies as well as in the presence of illness or disability in order to maintain life, provide comfort, reduce stress, and enhance the patients’ ability to cope
· May administer IV fluids if intravenous certification is current
· Cooperates with other personnel to achieve department objectives and maintain good employee, interdepartmental and public relations
· Performs other duties as assigned by Registered Nurse or DPCS
Qualifications
· Active and unencumbered Vocational Nursing License from the California Board of Vocational Nursing and Psychiatric Technicians
· IV Certification preferred
