GRANT SUMMARY
The goal of the Healthy Transitions project is to improve existing and expand access to developmentally, culturally, and linguistically appropriate services and supports for transition-aged youth and young adults (ages 16-25) who either have or are at risk for developing serious mental health conditions. The overarching goal is to improve emotional and behavioral health functioning so youth and young adults can maximize their potential to assume adult roles and responsibilities to lead full and productive lives.
POSITION SUMMARY :
This position is responsible for enrolling transition-aged youth and young adults into the Healthy Transition project and tracking consumers from referral through the project services until discharge.
JOB DUTIES
- Enroll consumers from the Screening, Brief Intervention, and Referral to Treatment service unit into the Healthy Transitions project.
- Collects National Outcome Measures (NOMs) using the NOMs matrix representing the beginning of a state-level reporting system that will create an accurate and current national picture of substance abuse and mental health services.
- Develops referral activities to meet project goals over the life of the project.
- Seeks out positive gathering activities and events where other transitional-aged youth and young adults congregate including street outreach, community and cultural events, school (one-on-one by appointment), and any other venues where the target population may be found.
- Identifies any emergency needs and transports them to emergency behavioral health services.
- Tracks all consumers referred through the outreach and social marketing campaign from referral to discharge and follows up to determine the success of the Healthy Transitions project.
- Conducts a warm transfer of the consumer from project introduction to the assessment unit for direction to the appropriate service unit.
- Ensures adherence to data collection requirements and enters accurate and timely data into the Electronic Health Record (EHR).
- Attends mandatory position training that provides training in risk factors as designated by the Project Manager and / or Clinical Supervisor.
- Maintains consumer confidentiality following 42 CFR Part 2 and HIPPA.
- Attend all required staff meetings.
- Performs other job-related duties as assigned.
