This is an exciting opportunity for a Licensed Practical Nurse (LPN) to provide comprehensive behavioral health nursing to adults and youth with mental health, substance use, and intellectual disabilities in a fast-paced and multi-culturally diverse medication clinic. Delivers therapeutic nursing and behavioral health interventions under the supervision of a registered nurse. Dually functions as a member of a nursing team and an interdisciplinary team. Provides direct nursing care to individuals including medication administration, vital signs, height, and weight measurements. Provides support and education regarding medical issues to non-medical team members. Using an electronic health record, will document all services provided. Perform health screenings and support the integration of behavioral and physical health.
Work hours : Monday through Friday 8 : 30am - 5 : 00pm. No holidays and no weekends. This position is site based with most duties delivered in the medication clinic.

Employment Standards
MINIMUM QUALIFICATIONS :
Graduation from an accredited Practical Nursing program; plus two years of experience as a Licensed Practical Nurse.
CERTIFICATES AND LICENSES REQUIRED :
CPR / First Aid certification
NECESSARY SPECIAL REQUIREMENT :
This position is emergency / essential services personnel. (Required to report for duty during inclement weather and / or other emergencies.)

PHYSICAL REQUIREMENTS :
Ability to sit, walk, stand, bend, cook, shop, drive and write as needed; use keyboard to enter data in electronic health record and review data on computer monitor. Ability to take vital signs (blood pressure, weight, abdominal girth, temperature, pulse, blood sugar finger sticks) and administer intramuscular injections and TB tests. Must be capable of providing nursing care and assistance including physically assisting large adults in hygiene. All duties performed with or without reasonable accommodations.
