NATURE OF POSITION
The Licensed Practical/Registered Nurse is an integral member of the interdisciplinary health team and assumes responsibility for the standard of patient care delivery in the clinic setting as directed by Fairfax Medical Facilities, Inc. policies and procedures.
QUALIFICATIONS
A. Licensed Practical/Registered Nurse with minimum of LPN or RN with an Associate’s Degree but a Bachelor Degree is preferred. Work experience will be considered for a licensed practical nurse.
B. Experience in ambulatory care environment.
C. Possess strong clinical and teaching skills.
D. Able to assume a leadership role in a team environment.
E. Strong interest in working with an interdisciplinary health team and culturally diverse patient population

RESPONSIBLE TO: CEO
SUPPORTIVE CHARGE RESPONSIBILITIES
A. Works in harmony with others exhibiting a positive attitude and a commitment to the basic mission of the health center.
B. Proactively assumes the responsibility for the standard of nursing care delivery and customer service in the clinical areas and serves as direct liaison between Provider/Administration/Front Desk and Other Support Staff/Patients.
C. Knowledgeable of the scope of practice for all nursing disciplines as described by the Nurse Practice Act.
D. Able to effectively plan, oversee, direct, and evaluate all nursing activities in the clinical area. Appropriately delegates tasks to Site Managers and other team members in order to maximize resources.
E. Assumes responsibility for and demonstrates ability to effectively organize and prioritize work and available resources to maintain efficient patient flow in the clinic and personally provides coverage when needed.
F. Assist the Site Managers with manipulating the nursing schedule as necessary, including floating coverage if needed, to maintain the best possible nurse/provider ratio, considering numbers, skill levels, cost effectiveness.
G. Supervises the completion of routine clinic tasks and establishes methods to ensure consistency in maintaining emergency supplies, stock medications, vaccines, forms, sterile instrument inventory, equipment, appropriate exam room inventory, etc.
H. Able to act as a primary resource in emergency situations.
I. Disseminates and collects operational information from team members in a timely manner.
J. Actions show commitment to team effort, works well with others, supports group objectives, and participates in staff meetings and team problem solving.
K. Effectively acts as preceptor to new nursing employees by assisting with the coordination of orientation schedule, orientation checklists, competencies, etc.
L. Demonstrates ability to identify clinic needs, develop and implement protocol, policy, and procedure changes for improvement.
M. Assists CQI Coordinator/Risk Manager to improve quality measures, outcomes, and documentation.
N. Responsible for oversight and management of activities including all Quality Controls, Policy and Procedures, and Processing in clinic labs and assurance of compliance in Lab.
O. Serves on Quality Assurance Committee.
P. Responsible for assisting with Risk Management activities.
Q. Knowledgeable of the procedures for Case Management and provide support as needed.
R. Knowledgeable of the procedures for Referrals and provide support as needed.
DIRECT AND INDIRECT PATIENT CARE/CLINIC RESPONSIBILIES
A. Follows protocols and performs accurate and thorough work-ups and check-ins of all patients.
B. Accurately assesses current patient status as well as preventive health status and communicates needs to provider.
C. Initiates and provides accurate patient education based on assessment of patient needs and clinic standards of nursing care, and documents in patient record.
D. Provides skilled nursing care as demonstrated by safely and accurately performing technical skills (sterile dressings, EKGs, pulmonary treatments, etc.) and the safe accurate administration of medications, IV fluids, and immunizations.
E. Anticipates the needs of providers related to patient visits and assist providers as needed.
F. Exhibits caring qualities consistent with the values of the clinic.
G. Assumes role of patient advocate to ensure health needs are met, to include responding to patient phone messages and requests in a timely and professional manner.
H. Documents all provider orders, nursing actions, and interventions on patient record consistent with guidelines, and utilizes approved abbreviations.
I. Appropriately triages patients on-site and via telephone for appropriateness and timeliness of care per approved triage protocols and standing orders.
J. Knowledgeable of and follows all established Infection Control guidelines and OSHA standards and participates in staff training events.
K. Performs approved laboratory testing, to include proper handling and labeling of all test samples, according to health center policy and procedure, and as allowed by CLIA guidelines.
L. Keeps patients and families informed of situations that affect the clinic visit; i.e. extended waiting times, seeing provider other than originally scheduled, etc.
M. Floats to any of the satellite sites as needed.
N. Performs other duties incidental to work.

ADMINISTRATIVE DUTIES
A. Supervises clinics when Site Manager is not available.
B. Creating, revising, planning and implementing policies & procedures.
C. Evaluating nursing staff and their competency skills, and complete staff annual performance evaluations as assigned.
D. Assist the CCO to implement Patient-Centered Medical Home recognition program achieving and maintain recognition for all clinics.
E. Supervise and monitor Incentive Programs and assist with Quality Assurance and Risk Management as a supportive role; implement necessary program processes to enhance clinic outcomes.
F. Performs other job-related duties as assigned.
