NATURE OF POSITION
The Social Worker is an integral member of the interdisciplinary team of the health center. The Social Worker will provide assistance for social problems in collaboration with the staff as well as the various service agencies in the county, but acts as a resource to all personnel.
QUALIFICATIONS
- LCSW currently licensed to practice in the state of Oklahoma.
- Graduate of formal accredited LCSW program and certified to practice by Oklahoma State Board of Social Workers.
- Board certified in any applicable specialty areas.
- Experience in social work and a clinical care setting.
- Ability to plan and execute work effectively and independently.
- Interest and/or experience in collaboration with interdisciplinary health teams.
- Possesses skills at a high level of competency.
SALARY CLASSIFICATION: Exempt
RESPONSIBLE TO: Behavioral Health Director
RESPONSIBILITIES
- Participates in the provision of first contact care and coordination of continuity of care to clients of all ages.
- Assumes responsibility for the care of a group of clients.
- Provides counseling with clients as indicated and works collaboratively with referral sources in providing Behavioral Health services to FMFI clients.
- Utilizes other service agencies in problem solving and makes appropriate referrals.
- Has understanding of the client and his/her problems, assesses his/her needs and plans solutions for problems with him/her. Maintains continued contact and reassesses the service offered to him/her.
- Participates in individual client or family education or counseling activities in collaboration with other members of the interdisciplinary team.
- Participates appropriately in staff activities and attendance in board meetings as requested.
- Assists with other clinic activities as needed, working with the director in planning and implementing programs.
- Documents appropriately according to policies and procedures using the EMR.
- Assists with overall coordination and administration of health care services and activities including clinical indicators, health care plan and quality assurance.
- May be required to travel to different FMFI locations.
- Participates in coordination of community agencies and/or resources in providing continuity of care.
- Conducts and/or participates in group educational experience for clients and/or families.
- Performs all job duties as described, which may be modified by FMFI from time to time at its sole discretion.
- Utilizes the EMR to document patient care and progress according to FMFI policies and procedures.
- Able to effectively communicate with the patient and if indicated family to establish a trusting relationship conducive to positive outcomes.
- Play an active role in obtaining targets of quality measures identified for Behavioral Health by the Continuous Quality Improvement Committee
