What are the responsibilities and job description for the Perinatal Cluster Coordinator position at Fairfield County Ohio?
Conducts service coordination that includes collaboration with medical and service providers to address health and well-being needs of families appropriately referred to the Perinatal Cluster Committee; seamlessly integrates treatment providers’ service plans; implements a structure that supports strong family engagement. Develops Plans of Safe Care for substance-exposed and substance-affected infants as required by the Comprehensive Addiction and Recovery Act.
Supports parents and caregivers through the Early Childhood Safety Initiative and Car Seat Programs. Educates and assists how to correctly secure children in appropriate child safety seats in multiple types of vehicles for families, parents, caregivers, and others transporting children. Participates in community events by performing car seat safety inspections. Provides evidence-based parenting services and support in the community via multiple formats (group, one to one, in-person, virtual)
Conducts communitywide data collection / tracks and shares results of collaborative efforts among all service providers to inform decision making and achieve better outcomes for families and children affected by SUD and other factors that may affect the health and well-being of new mothers and their infants. Coordinates communitywide data collection relative to Plans of Safe Care.
Increases collaboration with medical providers serving women and children. Conducts outreach to service providers and the medical community to inform of Perinatal Cluster services as well as federal requirements for Plans of Safe Care for substance-exposed and substance-affected infants.
Facilitates meetings of the Perinatal Cluster Committee and provides administrative support to the Perinatal Leadership Group toward achieving the community’s strategic objectives related to overall health outcomes for at-risk mothers struggling with challenges such as addiction, mental health concerns, lack of access to prenatal care, and homelessness. Coordinates technical assistance, training and cross-training for key staff, team members and community partners for developing Plans of Safe Care for substance-exposed and substance-affected infants.
Attends trainings required by FACF Council; Complies with the general operating standards of the FACF Council offices (cooperative work, fiscal and clerical deadlines/documentation, staff meetings, etc.); In conjunction with other team members and the FACF Executive Director, participates in problem solving and program fidelity; other duties as assigned.
Valid Driver's License
Bachelor’s degree in public administration, health administration, social services or closely related field
3-5 years’ experience in community resource settings
Knowledge, Skills, and Abilities:
Evidence of essential communication skills; Understanding of evidence-based practice; Highly organized with ability to keep accurate notes and records; Experience with health systems and reports is desirable; Experience collecting and analyzing data; Knowledge and understanding of Fairfield County human service community and resources; Group facilitation skills; Care coordination and/or case management experience; must have completed or be willing to complete within the first six months of service Screening, Brief Intervention and Referral to Treatment (SBIRT) training and Trauma Certification.
Must have good mediation and collaboration skills; Ability to effectively develop rapport with social service organizations, court and healthcare personnel, vendors and the public; Ability to effectively document services; Strong facilitation skills; Ability to work effectively with multi-disciplinary teams; Ability to utilize critical thinking skills; Ability to implement planning across disciplines and time; Ability to be proactive in problem-solving situations.
Knowledge of data gathering and analysis techniques; Skilled in writing and public speaking; Ability to work independently with and facilitate the work of committees and agency professionals. Ability to use MS Office Suite.
