Job Title : Rehabilitation Therapist

Job Specialty : Physical Therapist

Job Duration : 13 weeks

Shift : Monday-Friday, Days

Guaranteed Hours : 32

Experience : New graduates accepted; SNF experience highly preferred

License : State license verification required

Certifications : 8 Hours of Dementia Training

Must-Have :

Completed and attached prescreen documentation
Resume
License verification
Three references

Job Description :

Perform patient assessments and develop individualized treatment plans

Implement therapeutic exercises and modalities to promote the rehabilitation of patients

Document and track progress towards patient goals

Collaborate with a multidisciplinary team to optimize patient care

Adhere to compliance standards including county, federal, multi-state criminal searches, and national sex offender searches

Participate in HHS / OIG, GSA, and SAM searches

Maintain updated knowledge and application of clinical skills

Complete background checks including Social Security Number Trace and CORI Massachusetts Background

Ensure 1 Step PPD or Quantiferon or proof of a positive PPD and chest X-Ray is current
