General Position Description :
The Licensed Vocational / Practical Nurse performs nursing care for patients of all ages in their place of residence, coordinate care with the Interdisciplinary Group / Team, patient / family, and referring agency; and contributes to the coordination of care. The Licensed Vocational / Practical Nurse reports to the Hospice Administrator.
Role Expectations :
- Under the direction of the RN, assists in identifying the patient’s physical, psychosocial, and environmental needs as evidenced by documentation clinical record, team report, and evaluations.
- Participates in planning and implementing care in conjunction with the RN, delivers and documents care based on the Interdisciplinary plan of care
- Provides nursing care utilizing infection control measures (OSHA)
- Assures the continuity of care through delivery of quality patient care.
- Work with the RN and the Interdisciplinary Group / Team to ensure patient / family needs are met.
- Communicate with attending physicians, hospice physicians, and other physicians involved in the patient’s care.
- Timely submission of all required paperwork.
- Demonstrates commitment and professional growth by participating in hospice sponsored in-service programs and maintaining or improving competency. Participates in the Hospice’s QAPI program.
- Promotes the Hospice’s philosophy and administrative policies.
- Meets productivity standards.
- Performs on-call responsibilities and provides on-call services to patients and their families as assigned.
- Provides effective communication to patients, their family members, team members, and other health care professionals.

Requirements :
Education and experience :
Two (2) years' experience as a Licensed Vocational / Practical Nurse in a clinical care setting and one (1) year full time hospice.
Education :
Graduate of an accredited Diploma or Associate School of Nursing.
Licensure :
Current State license as a Licensed Vocational / Practical Nurse; Current driver’s license
Skills :
Nursing skills as defined as generally accepted standards of practice. Knowledge of state and federal regulations. Good interpersonal skills
