Now Hiring: Registered Nurse Case Manager Full Time

$2,000 Sign-On Bonus for Full-Time Nurses

Essential Job Functions Include:

• Manage and provide case management services to LHCSA clients, ensuring their needs are met with a personalized care approach.
• Update and monitor Plans of Care regularly and ensure compliance with regulatory guidelines.
• Complete UAS assessments every 6 months and annually, and make sure each resident's needs are addressed effectively.
• Support residents and families with the orientation to the facility and new routines.
• Provide supervision and leadership to Home Health Aides (HHA) and Personal Care Aides (PCA).
• Coordinate transportation and arrange needed community services for residents.
• Monitor medication audits, review orders, and renew necessary prescriptions.
• Assist with maintaining residents' family and social support networks.
• Act as a liaison between residents, families, healthcare teams, and community agencies.
• Participate in comprehensive care planning, ensuring the best possible care for each resident.
• Handle on-call rotation as required and maintain a cooperative and positive team dynamic.
  
  
  

Requirements:

• Active New York State Registered Nurse (RN) License.
• Experience in geriatric nursing preferred.
• Knowledge of ACF/ALP regulations is a plus.

• Must be organized, detail-oriented, and able to lead and support a team.
• Strong communication skills and ability to work collaboratively with other staff and residents.
• Familiar with regulatory agencies and compliance standards.
• Flexibility and the ability to adapt to evolving needs and priorities.
