What are the responsibilities and job description for the Social Worker position at Fairview Community Health Center?
DUTIES AND RESPONSIBILITIES INCLUDES, BUT IS NOT LIMITED TO:
- Determines nature of patient’s situation by interviewing for Social Determinants of Health (SDOH) and establishing rapport.
- Build a plan of action by exploring options and setting goals with the patient.
- Obtain assistance for patient by referring him/her to community resources (mental health, substance use, transportation, etc.).
- Connect with other agencies for collaboration on education, outreach, and patient assistance.
- Maintain accurate and timely documentation, including periodic follow-ups.
- Participate in quality measures, report issues, and monitor patient outcomes.
- Keep information confidential and follow clinic, local, state, and federal policies and guidelines.
- Coordinate implementation of Community Health Worker (CHW) program and comply with grant requirements; mentor team and limitations of the CHW role.
- Facilitate administrative tasks for clinic departments.
- Participate in continuing education programs, in-service education programs, and peer support system with other Community Health Workers.
- Any other duties as assigned.
Education, Training, Knowledge and Experience:
- Current LCSW
- Bachelors Degree in Human Services Field (Social Work, Psychology, sociology, etc)
- Experience working with diverse vulnerable populations (for example: refugee/immigrant, substance use, homeless, developmentally disabled)
- Experience in human services; familiarity with case management
- Basic medical terminology knowledge
- Basic computer skills
