Casual Cardiac/Pulmonary Rehab Therapist

We are seeking a casual cardiac/pulmonary rehab therapist to join our Rehab team at Fairview Lakes Medical Center!

This position includes cardiac rehab, pulmonary rehab and PAD rehab and will have direct patient involvement in both 1:1 and group settings. The therapist will work with patients during their evaluations and discharge sessions; as well as, provide group education and 1:1 individual education. The therapist will also assist with developing and updating a plan of care; using critical judgment to determine if patient's care team needs to be contacted (related to high blood pressure, angina symptoms, etc.). In collaboration with patients and families, the cardiac rehab therapist is responsible for providing safe and effective delivery of patient care within scope of practice. This includes examination, evaluation, diagnosis, planning, intervention and establishing outcomes.

M Health Fairview Rehabilitation offers a broad range of services that serve patients across 10 acute-care hospitals, 3 post-acute settings and 70 outpatient adult and pediatric therapy clinics. Consisting of Physical, Occupational and Speech Therapy as well as Audiology and Cardiac & Pulmonary Rehab, our therapists collaborate with colleagues in all medical settings and offer dozens of specialty programs.

The cardiac rehab therapist partners with the patient and family while performing the Evaluation/Re-evaluation

Gathers pertinent data

Performs examination

Determines treatment diagnosis

Develops an individualized plan of care through collaboration with the patient and family

Analyzes data to identify needs and potential risks

Determines prognosis for recovery

Establishes individualized goals with patient and/or caregiver

Individualizes plan of care considering developmental level and cultural differences

Implements and modifies plan of care based on reassessment and patient response

Coordinates care and communicates effectively with interdisciplinary team for all aspects of patient care

Establishes appropriate discharge plan

Partners with patients and families to provide skilled intervention

Implements plan of care

Alters treatment/plan of care to reflect change in patient status and response to treatment

Assesses patient and/or caregiver learning needs and provides appropriate education

Documentation/Billing

Uses acceptable medical terminology

Follows professional practice standards

Completes documentation in a timely manner per department standards

Complies with department billing standards

Required

Bachelor’s Degree in exercise-related field (Exercise Science or Exercise Physiology), or

Current Minnesota licensure as a:

• Physical Therapist, or
• Occupational Therapist, or
• Registered Nurse

Basic Life Support Certification

Advanced Cardiac Life Support Certification (within 6 months of hire) 

Preferred

Cardiac Rehab Internship 

1 year(s) experience

Certified Exercise Specialist (CES)

Registered Clinical Exercise Physiologist (RCEP)

Certified Clinical Research Professional (CCRP)

Master’s degree in Nursing or Physical Therapy or Occupational Therapy, or an exercise-related field (Exercise Science, Exercise Physiology)

An individual's pay rate within the posted range may be determined by various factors, including skills, knowledge, relevant education, experience, and market conditions. Additionally, our organization prioritizes pay equity and considers internal team equity when making any offer. Hiring at the maximum of the range is not typical.
