M Health Fairview has an immediate opening for an LPN at the Wyoming Lakes Specialty Clinic located in Wyoming, MN. This role is approved for 48 hours every two weeks and is benefit eligible. Responsibilities Job Description
The LPN performs delegated nursing and medical functions of data collection and delivery of care to an assigned group of patients, or individualized patient. The LPN works under the direction of a Registered Nurse, or physician/authorized practitioner and in collaboration with other members of the healthcare team. The LPN is responsible for adhering to all policies, procedures, and nursing practice guidelines.
- Involves the patient, family, and other health team members in gathering relevant data.
- Documents data collected in the medical record.
- Delivers care as delegated by the RN or physician/authorized practitioner to assigned patient population, or individual patient
- Follows the patient’s plan of care.
- Prioritizes work based on patient acuity, available resources, and patient/family preferences, in collaboration with the RN or physician/authorized practitioner.
- Performs high and low risk technical procedures, including medications, defined by the clinical setting.
- Communicates effectively with other members of the health care team.
- Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served.
Qualifications
Required
Vocational/Technical Training or Associates Degree
One year previous healthcare and/or customer service experience
Current Minnesota LPN License
Current BLS certification
Preferred
One to three years of previous LPN experience
Previous ambulatory care experience
Other Skills We Desire:
