We at Fairview Health Services are seeking for a driven Licensed Practical Nurse (LPN) to join the hard working Women's Health team at our Maple Grove Ambulatory Care Center. We have approved this position for 80 hours every two weeks and it is benefit eligible! Responsibilities Job Description

The LPN performs delegated nursing and medical functions of data collection and delivery of care to an assigned group of patients, or individualized patient. The LPN works under the direction of a Registered Nurse, physician and/or authorized practitioner and in collaboration with other members of the healthcare team. The LPN is responsible for adhering to all policies, procedures, and nursing practice guidelines.

Collects data for patient assessments:

  • Involves the patient, family, and other health team members in capturing relevant data
  • Document’s data collected in the medical record
  • Reports unusual data or changes in patient condition to the RN, or authorized practitioner in a timely manner

Delivers care as delegated by the RN or physician to assigned patient population, or individual patient:

  • Follows the patient’s plan of care
  • Prioritizes work based on patient acuity, available resources, and patient preferences
  • Recognizes when different approaches may be needed based on age or cultural differences
  • Implements assigned aspects of the patient teaching plan
  • Documents care into medical records/EMR
  • Effectively communicates compliance and concerns to delegating RN/physician/authorized practitioner
  • Takes action to assure continuity of care for all assigned patients.
  • Responds appropriately to emergent situations

Communicates effectively with other care team members:

  • Communicates verbal and written information in a clear, concise, accurate, and timely manner
  • Uses communication approaches appropriate to patient/support network needs

Qualifications

Required

  • Vocational/Technical Training or Associates Degree
  • Current LPN License

Preferred

  • One to three years
  • Previous experience working in a clinic or in OBGYN
  • Current BLS Certification
