Fairview Range is hiring a Community Health Workerfor our Behavioral Health Home Department in Hibbing, MN.
The Community Health Worker (CHW) is responsible for helping Behavioral Health Home patients, and their family’s access and navigate community services and resources and adopt healthy behaviors. The CHW works with the BHH Social worker through a coordinated approach to care management and community outreach. Activity will promote, maintain and improve individual and community health identified in the patient treatment plan. The CHW provides social support and informal counseling, individual health education and advocacy for individuals and community health needs. Providing home visits and in clinic visits as determined by the CHW and BHH team required.
Schedule : This position will work 72-80 hours per two weeks. The shift length will be 8-hours on the shift. Weekends : None
