Responsibilities Job Description
This position provides nursing care to patients with cardiovascular disease within the scope of practice for a registered nurse. Care is delivered in collaboration with the physicians and healthcare team. Evaluates patients, reviews diagnostic tests and initiates/modifies diet, lifestyle, and other therapies using nursing processes.
Job Expectations:
- Consistent, ongoing monitoring, assessment, documentation and personalized education of patients with attention to detail and focus on the patient experience.
- Coordination of care with a variety of disciplines and facilities, to maximize patient outcomes and minimize recurrent hospitalizations.
- Case Management-style Nursing model with goal-directed care consistent with high quality care.
- Facilitate Patient Assistance Medication Programs or Grants available to those meeting guidelines Advocate/assist in referral process for appropriate therapies.
Qualifications
Required
Education:
Associate degree in Nursing required.
Experience:
2 years of post-coronary or coronary critical care experience as a registered nurse.
Knowledge:
Knowledge of adult cardiovascular health and disease processes as well as related diagnostic and treatment modalities. Ability to perform a brief history and physical examination and to recognize abnormal findings.
License/Certification/Registration
Basic Life Support (BLS)
Registered Nurse MN (RN)
Preferred
Education:
Bachelor's degree preferred
Experience
1 year of Cardiac Telemetry or ICU experience as a registered nurse.
Knowledge:
-Strong knowledge of adult cardiovascular health, especially heart failure, and related diagnostic and treatment modalities. Ability to perform a brief history and physical examination and to recognize abnormal findings. Bachelor's degree preferred
-IV insertion skills
License/Certification/Registration:
Willingness to obtain nursing certification within 1 year of employment
