Do you have a current Minnesota Pharmacist license with at least 1 year experience as a practicing pharmacist? We at MHealth Fairview are looking for you to join our amazing team in Minneapolis! This benefits eligible position is a 1.0FTE(80 hours / 2 weeks) on the day shift with rotating weekends!
Assist the pharmacy manager in the operation and management of the pharmacy, provide clinical care to patients of the pharmacy, provide indirect supervision for the support staff and ensure adherence to all policies and procedures Specifically this role would be supporting the Advanced Therapies Clinic.
Provide appropriate pharmaceutical care including patient counseling and prospective drug use evaluation to all patients receiving home IV therapy.
- Perform initial patient evaluation as it pertains to the prescribed therapies. Collect, analyze, interpret, and evaluate appropriate data relative to medication therapy. Obtain histories and apply these along with other patient data to optimize drug therapy management. Work with other healthcare providers to evaluate patients to determine appropriateness for home IV therapy.
- Evaluate the significance of signs and symptoms relative to disease processes and makes recommendations for the modification of drug therapy.
- Evaluate the significance and clinical importance of adverse reactions and complications associated with drug therapy.
- Supervise patient records for potential drug / drug or drug / food interactions as well as patient allergies.
- Provide counseling and answer questions from patients, family and health care providers concerning drug therapy.
Provide prescription drug therapy to patients / customers ensuring accurate labeling and dispensing of medications in a timely fashion.
Ensure dispensing of prescriptions in accordance with all legal requirements, regulatory agency and Fairview Home Infusion Policy and Procedures.
Maintain proficiency in the skills and theory of the practice of pharmacy by reviewing literature, attending in-services, seminars and conferences
Required : Education :
High School Diploma / G.E.D. B.S. Degree or Pharm D in Pharmacy from an accredited school of pharmacy
License / Certification / Registration
Current Minnesota Pharmacy License
Experience :
One - three years experience as a Practicing Pharmacist
