Responsibilities Job Description
This position is responsible for providing clinical support and service coordination for patients on an inpatient mental health unit at Fairview Riverside hospital. This role serves a critical function working with a multidisciplinary team to meet patient needs. This includes scheduling patient’s for appointments and transportation, discussing follow-up plans with patients, coordinating with other providers and services, completing appropriate paperwork and documentation, and assisting licensed clinicians as needed.
A variety of hours are available and there is an opportunity to do some remote work.
- Assesses and triages patients to be seen by licensed mental health professionals
- Secures information pertinent to the patient encounter.
- Obtains internal and external records when possible and coordinates care for patients
- Schedules appropriate appointments for patients and refers to other programs or community agencies when Fairview services are not appropriate.
- Escalates triage and placement concerns appropriately
- Coordinates and facilitates all inquiry calls regarding services; uses excellent telephone etiquette and active listening skills.
- Provides case management support
- Completes documentation in a clear and timely manner
- Consults with appropriate resources to meet patient needs.
Required
Education
· Bachelor’s Degree in or 6 years directly related experience in Human Services related field.
Experience
· 2 years clinical experience in Behavioral Field
Preferred
Education
· Master’s Degree in Behavioral related field
Experience
· 3-5 years clinical experience in Behavioral Field
License/Certification/Registration
· Crisis intervention training.
Qualifications
$27.34-$38.59 / Hourly
