What are the responsibilities and job description for the Manager Inpatient Care Management RN position at Fairview Health Services?
This position provides leadership for the development, engagement, management, performance and ongoing improvements to their assigned teams at M Health Fairview. Responsibilities include staff within Acute Care Hospitals, including but not limited to Clinical Social Workers and RN Care Managers. This individual plays an integral role in enhancing clinical integration through operationalizing care management strategy, goals and outcomes across assigned acute care responsibilities by building relationships and collaborating to achieve standardized and evidenced based practices that promote quality care and services to patients. Accountable for daily throughput of patients through planning, organizing, supervising, monitoring and coordinating the efforts of the team utilizing the daily engagement system and lean principles.
Facilitates and achieves financial outcomes through effect and efficient staffing, scheduling, consistent team performance through quality services, resource utilization and management of Care Management teams.
Participates in patient-centered development of high performing care management department collaborating with all disciplines including community and payers with focus on tangible outcomes related to improved quality, patient, provider, staff satisfaction, and decreased costs. Oversees, the facilitation of exceptional care transitions across the patient continuum with positive contribution to the overall patient experience. Responsibility for hospital utilization performance improvement and operations management inpatient Acute Care Hospitals to promote effective utilization of hospital resources, appropriate reimbursement for services rendered, support efficient patient throughput and ensure compliance with all state and federal regulations related to care management and utilization services. Manages the daily operations of care management to meet department objectives of throughput, determining medical necessity (admissions, discharge, observation and post procedure care), coordination of appropriate plans of care, discharge planning, documentation, referrals, and care facilitation across continuum. Actively participates in identifying, creating and implementing improvements to achieve clinical, satisfaction, efficiency and operational outcomes. Proactive utilization management. Directs department human resources ensuring proper utilization of human resources and positive employee relations. Set performance standards aligned with commitment to growth and development, reviews performance, and provides coaching, feedback and meaningful recognition on job performance on an ongoing and timely basis.
Develops others so they become well versed in lean management and performance improvement methods through knowledge transfer and adoption of lean thinking in strategy deployment and daily engagement system. Provides leadership to foster a culture of improvement, efficiency and innovative thinking. Manages data, maintains knowledge of, and complies with all relevant laws, regulation policies, procedures and key performance indicators to evolve care management services.
