This position is a new growth position to offer PHP and IOP services at our Edina location.

Responsibilities Job Description

Provides assessment, education and therapeutic interventions related to health/wellness issues for clients and their families. Serves as a liaison between the physician and the interdisciplinary team. Effectively communicates verbally and in writing with the interdisciplinary team and external support systems. Participates in the development and implementation of the treatment and discharge plan. Functions as an interdisciplinary team member. Responsible, as assigned, for the management of caseload.

 

In addition to the above, you will be responsible for the following:

 

Works collaboratively with the physician and the interdisciplinary team.

Contributes to the development of an individualized treatment plan.

Participates in developing and implementing a clinically appropriate discharge plan.

Facilitates individual and group psycho-education.

Communicates effectively with others.

Demonstrates the ability to support the business functions of the outpatient program.

Demonstrates the ability to advance evidence-based practice by utilizing standards, guidelines and pathways for care delivery. Incorporates data and information into practice to continually improve care and practice so that patient outcomes are enhanced.


Qualifications

Required

 

Education: Bachelor’s Degree in Nursing or Behavioral Science

Experience: Three years in mental health or related setting

License/Certification/Registration: Current Registered Nurse license in Minnesota, Basic Life Support Certification prior to end of orientation

 

Preferred

 

Education: Bachelor’s Degree in Nursing or Behavioral Science

Experience: Outpatient experience

License/Certification/Registration: Certified Registered Nurse

Additional Requirements (must be obtained or completed within a period of time):

Basic computer and keyboarding skills

For Nurse Liaisons in Behavioral, completion of de-escalation and self-defense training and retraining every two years or as often as required. (Not required for Fairview Medical Group Nurse Liaisons.)

Basic Life Support Certification prior to end of orientation
