Overview
We are seeking a physical therapist to join our outpatient rehab facility within the M Health Fairview Lakes Medical Centers location in Wyoming, MN! Our ideal candidate has a strong orthopedic treatment skill set. The patient population at this clinic includes general practice PT treating primarily orthopedic diagnoses. Other opportunities to practice in specialty areas including neurological, vestibular, concussion, cancer rehab, and chronic pain are available. This position will have a small role in supporting treating acute care patients on the medical floor when volumes dictate and the outpatient schedule allows. In collaboration with patients and families, the physical therapist is responsible for providing safe and effective delivery of patient care within scope of practice. This includes examination, evaluation, diagnosis, planning, intervention and establishing outcomes.
- FTE 1.0, authorized to work 80 hours per pay period
- Schedule: Monday - Thursday, 10-hours days, Fridays off

M Health Fairview Rehabilitation offers a broad range of services that serve patients across 10 acute-care hospitals, three post-acute settings and 70 outpatient adult and pediatric therapy clinics. Responsibilities/Job Description
The physical therapist partners with patients and their families while performing evaluation and re-evaluation
Develops an individualized plan of care through coordination
Capture and apply pertinent data, performs examination, and resolves treatment diagnosis
Analyzes data to identify needs and potential risks, settles prognosis for recovery, and establishes individualized goals
Implements and modifies care plan based on reassessment and patient response
Coordinates care and communicates effectively with interdisciplinary team for all aspects care & establishes appropriate discharge plan
Partners care team to provide skilled intervention
Implements and alters treatment to reflect change in patient status and response to treatment
Assesses patient and/or caregiver learning needs and provides appropriate education
Documentation/Billing
Uses acceptable medical terminology and follows professional practice standards.
Completes documentation in a timely manner per department standards
Stays in sync with department billing standards
Professional Expectations
Prioritizes tasks and patient care for effective time management.
Understands and follows professional licensure/certification requirements
Advises team members and/or supervises students as designated

Qualifications
Required License/Certification/RegistrationPhysical Therapy registration/licensure in the state of Minnesota or temporary license in MNCurrent Basic Life Support (BLS) certification
Preferred Education & ExperienceDoctorate degree in Physical Therapy1 year of Physical Therapy practice
