What are the responsibilities and job description for the Supervisor; MTM Pharmacy - Pharmacist position at Fairview Health Services?
Pharmacy supervisor of the Specialty MTM Pharmacists working in hospital-based clinics.
Pharmacy supervisors have accountability for leading high-functioning pharmacy teams and supervising daily pharmacy services in their designated area(s). This includes, but is not limited to, medication therapy management, regulatory compliance, drug information, medication safety, process improvement, clinical service support, and education/development of staff, students or residents.
Job Expectations:
- Manage the day-to-day operations of MTM Practitioners including care coordination, staffing, documentation, and communication.
- Provide direct supervision of the MTM Practitioners.
- Oversee day-to-day operations to ensure appropriate distribution of workload and streamlined workflow.
- Assure completeness and accuracy of documentation and consistency among practitioners.
- Maintain and monitor the productivity level of the staff to maximize efficiency, assure adequate staffing levels, and control costs; approve timecards.
- Complete timely performance reviews of staff according to policy.
- Assist with the operational components for the overall program.
- Assist with recruitment/interview/hiring of new staff. Conduct orientation of new staff, and assist in ongoing education of current staff.
- Assist with management of costs within projected budget.
- Actively participate in department planning and implementation of those plans.
- Maintain effective and timely communication with MTM Operations Management.
- Provide Medication Therapy Management to patients as outlined in the MTM practice model.
- Ensure all drugs are appropriately indicated, effective, safe, and convenient for the patient.
- Obtain histories and apply these along with other patient data in the clinical evaluation.
- Identify, resolve and prevent medication therapy problems.
- Evaluate the signs and symptoms relative to disease processes and make recommendations and/or modifications of medication therapy utilizing collaborative practice agreements developed and approved by the Fairview MTM program and M Health Fairview system.
- Evaluate the significance and clinical importance of adverse reactions and complications associated with drug therapy.
- Ensure that the patient understands the specific goals of therapy and that the patient and providers are working to achieve these goals.
- Evaluate and document patient outcomes at each encounter, as required by the MTM practice model.
- Maintain proficiency in the skills and theory of the practice of pharmacy by reviewing literature and attending in-services, seminars and conferences.
- Actively participate in department planning, goal setting, and implementation of those goals/plans. Actively participate in MTM and clinic meetings.
Practice Involvement:
- Maintain proficiency in the skills and theory of the practice of pharmacy by reviewing literature, attending in-services, seminars, and conferences.
- Lead and participate in the MTM clinical meetings.
- Assist with Practitioner development- ensuring awareness and implementation of new protocols, guidelines, and policy and procedures

Minimum Qualifications to Fulfill Job Responsibilities:
Required
Education
- Bachelor’s Degree in Pharmacy and/or Doctor of Pharmacy Degree. Certification in Pharmaceutical Care as approved by the Fairview Pharmacy Services Management
Experience
- 2 or more years as a practiting MTM practitioner
License/Certification/Registration
- Licensure as a Registered Pharmacist in Minnesota
Preferred
Education
- Successful completion of an ASHP-accredited PGY1 residency program, ideally in ambulatory care
Experience
- 1 or more years of pharmacy leadership experience

The posted pay range is for a 40-hour workweek (1.0 FTE).
