We at Fairview Health Services are seeking a casual Transplant Coordinator to be based on the University of Minnesota - East Bank Campus. This is a casual position meaning that all hours will be as needed. Pay for this role starts at $44/hr and increases based on experience.
As one of the nation’s highest volume transplant centers, M Health Fairview’s transplant program continues to deliver outstanding expertise and innovative technologies. Located on the U of M campus we are proud to say that our Transplant Team is by far the best in the business. While working in this as-needed role, the coordinator will review the inboxes in Epic and MyChart, respond to any urgent requests and deescalate emergency situations. The manager for this department states "This is an outstanding role because the main responsibility is to make sure there aren't any fires to be put out." You will also be able to connect with other teammates via Microsoft Teams if you have questions during your shift. We only have adult patients after they have had their procedures, limiting the type of inquires you will experience.
Responsibilities Job Description
The Transplant Coordinator are responsible for coordination of the long-term management of patient’s health and wellbeing after transplant through collaboration with various members of the interdisciplinary team. They will collaborate with interdisciplinary team members within the organization and outside community partners while also acting as a liaison for transplant physicians to implement medical management of conditions. You may be asked to provide telephone triage and make patient care decisions based on patient symptom assessment and evaluation, laboratory results, and patient condition changes. We believe one of the most exciting responsibilities Transplant Coordinators has is educating the patients and their caregivers.

Qualifications
Required
- BA or BS in Nursing and 3-5 years acute care experience in ICU, CCU, Transplant, Acute Care Med-Surg
- OR-
- AD in Nursing and 5 years of acute care experience in ICU, CCU, Transplant, Acute Care Med-Surg
- MN RN license
- BLS
