M Health Fairview has an immediate opening for a Visit Facilitator to join our team at the BMT Program Office. Join M Health Fairview, the largest healthcare system in the state! 

Work with Nurse Clinican to get patient edcations material/binders setup, scanning into EPIC, making patient reminder phone call, running workbench reports in EPIC (will provide very detailed training)

We needed someone who is independent and detailed oriented. We provide excellent training but need someone who can use their resources to figure out solutions to new suitations. Some who has prior EPIC experience would be a plus. 

Needs very good attention to details, good communicator, on time to work, Timely with tasks and commuincates when tasks cant be completed.  

This is a 1.0 FTE (80 hours per two week pay period) opening. The tentative schedule for this opening is Monday - Friday 8AM - 5PM, subject to change based on clinic needs.

The M Health Fairview Clinic – BMT Program Office is seeking to hire a Visit Facilitator to join our Speciality Care team.  Our clinic utilizes a team-based approach to support our Internal Medicine and Family Medicine providers in delivering excellent care to our patients and community.  Visit Facilitators are a valuable member of our care team and collaborate with providers and support staff in meeting patient care needs during and between appointments in the clinic.  Visit Facilitators on our team gain experience beyond patient care by working on collaborative projects with other clinics in our service line, leading process improvement initiatives in our clinic, serving as subject matter experts in the clinic, and receive ongoing competency and skills training by our clinical leads.  Our team is committed to delivering excellent care to our patients, embracing diversity, being a welcome environment, and finding opportunities for personal and professional growth. 

This position is responsible for supporting in person clinic visits & helping guide patients through the care experience. Visit facilitators capture pertinent medical information from the patient & details the information into the medical record. This role also helps ensure the exam rooms are stocked, equipment & supplies are available, assists with tasks that support the effectiveness & efficiency of the team. The Visit Facilitator may also assist in scheduling & coordinating appointments, preparing schedules & other duties as assigned.

• Prepare patients for their appointments including: communicating prior to their scheduled appointment to ensure medical records are collected, received & available for visit
• Assist in rooming process by collecting & detailing pertinent information: reason for visit, medication review, vitals collection with automated vitals machine, & health questionnaires
• Supports the patient connection to the provider for the visit & acting as a resource while ensuring a positive patient/provider experience
• Completes visit wrap-up/after-visit summary is provided & patients are aware of recommendations for follow up. This may include assisting with scheduling & coordinating follow-up appointments.
• Communicates efficiently with patients, providers & care team, demonstrating comfort in person & with virtual technologies

Required

• At least 1-year of customer service experience
• Proficient with technology (computer hardware and programs, smart devices such as phones or tablets)

Preferred

• Current nursing, medical, social work, psychology, or pharmacy student
• Successful completion of: NA, EMT, or HUC program
• Experience with Medical terminology, Epic Health Records, Telephonic customer experience and has previously worked in a health-care setting
