What are the responsibilities and job description for the Visit Faciliator-In Clinic position at Fairview Health Services?
This position is responsible for supporting in-person clinic visits. The Visit Facilitator prepares examination/procedure rooms before each patient, maintaining environmental services and infection control practices. This role helps to prepare each patient for their visit by gathering and documenting pertinent patient reported medical information. The Visit Facilitator also helps with scheduling and coordinating appointments, preparing schedules and office visits and other delegated communication.
Responsibilities Job Description
- Prepare examination and procedure areas for patients appointment
• Set-up the exam or procedure room
• Stock supplies
• Clean each exam/procedure room before and after each patient visit, in accordance with
environmental service and infection control practices.
• Prepare for proceudures by briniging in and setting up equipment or supplies.
• Transport and clean equipment, instruments, etc. - Initiating visits with the patient, collecting and documenting pertinent information such as
the reason for their visit, health history, patient reported medication review, vitals collection
with automated vitals machine, and applicable health questionnaires. - Completes visit wrap-up by scheduling and coordinating follow-up appointments and other
needs.

• Other administrative duties as assigned - Performs other duties including Performance Goals developed by manager and employee and
reported in the employee’s Performance Review as part of UMPhysicians’ Performance

• Maintains core/universal competencies and completes any new required learnings.
• Demonstrates all critical competencies and new required learnings.
Qualifications
Required: High School Diploma or GED
Preferred: Nursing Assistant, Medical or Nursing Student
Required:
• At least 1-year of customer service experience
• Proficient with technology (computer hardware and
programs, smart devices such as phones or tablets)
Preferred:
• Medical terminology
• Epic Health Records
• Telephonic customer experience
• Previously worked in a healthcare setting
