Responsibilities:
- Conduct initial assessments and develop individualized treatment plans
- Deliver physical therapy interventions to help residents improve mobility, strength, and function
- Monitor and document patient progress and adjust care plans as needed
- Communicate with nursing staff, family members, and interdisciplinary team
- Ensure high standards of care and compliance with all state/federal regulations
- Current licensure as a Physical Therapist (PT) in the state of Pennsylvania
- Experience in a skilled nursing or geriatric care setting preferred
- Strong interpersonal and clinical skills
