Job Title : Area Manager
Job Overview :
The Host Home Manager is responsible for overseeing the daily operations of the host home program, ensuring that individuals with disabilities or other specialized needs receive the care, support, and services necessary for their health, safety, and well-being. The Manager ensures that all staff, caregivers (hosts), and members comply with state regulations, policies, and procedures.
Key Responsibilities :
- Supervision of Host Homes :
- Recruit, train, and support host home providers who offer personal care and supervision in a home environment.
- Conduct regular home visits to assess the quality of care, safety, and comfort of members.
- Ensure that hosts are properly trained in De-Escalation, Medication Manager, Positive Behavior Supports, Rights and Restrictions etc.
- Supervision of Group Home :
- Recruit, train, support staff, support members in the group home.
- Conduct regular visits to the home, assess quality of care, safety and comfort of the members.
- Conduct routine checks on staff ensure they are following HCBS setting rule, have proper training and support to provide care for the members.
- Complete scheduling for all staff in the home.
- Hold monthly staff meetings and trainings.
- Hold member meetings for monthly activities, meal planning etc in the home.
- Be on call for staff and members and available should someone call off. This person will be responsible for finding staff to cover open shifts and may have to work the floor to cover any unfilled shifts as needed.
- Attend quartely meetings and annual PCSP meetings.
- Individualized Support :
- Collaborate with care teams, families, and individuals to create and implement individualized service plans (ISPs) based on the needs and preferences of members.
- Ensure that the service plans are regularly reviewed and updated as needed.
- Monitoring and Compliance :
- Monitor compliance with applicable laws, regulations, and agency standards for host homes.
- Complete regular documentation including assessments, incident reports, and member progress notes.
- Respond to any emergencies or incidents, providing immediate support and ensuring proper documentation is completed.
- Quality Assurance :
- Perform routine audits of host homes to ensure compliance with health, safety, and behavioral standards.
- Address any deficiencies or concerns identified during audits and follow up to ensure they are resolved.
- Communication and Coordination :
- Serve as a primary contact for host homes, members, and their families.
- Facilitate effective communication between host homes, members, family members, service providers, and any involved agencies.
- Address any concerns or grievances raised by members, families, or hosts, providing solutions or referrals as needed.
- Administrative Duties :
- Maintain accurate records for each host home and member, ensuring documentation is up-to-date and compliant with regulatory requirements.
- Assist in budget management and financial oversight related to the host home program.
- Track and report on program performance and outcomes.
Qualifications :
