Under the supervision of a registered nurse, the LPN provides quality care to patients and performs technical skilled care in various hospice settings in compliance with nursing standards, federal, state, local government regulations and Faith & Family Hospice clinical policies.
Job Responsibilities
- Provides direct and individualized nursing care to assigned patients based on nursing standards and under the supervision of the Clinical Director of Nursing
- Ensure quality and safe delivery of hospice services to patients and families/caregivers
- Implements plan of care formulated by physician and care plan
- Maintains safe and healthy working environment and uses safe work methods and procedures in accordance with Faith & Family Hospice clinical standards
- Participates in coordinator of services with Registered Nurse and clinical management
- Assists patient in learning appropriate self-care activities
- Documents and reports activity as required
- Responsible for training CNA's and Licensed Practical Nurses on specific clinical tasks
- Participates in admission process and on-call responsibilities
- Performs other reasonable related duties as assigned by management, including but not limited to development of an LPN training curriculum
