Overview
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. The Registered Nurse Case Manager will work closely with patients, families, and healthcare providers to coordinate care, manage medical records, and facilitate discharge planning. Responsibilities
- Assess patient needs through comprehensive evaluations and interviews.
- Develop individualized care plans that address the physical, emotional, and social needs of patients.
- Conduct thorough medical record reviews to ensure compliance with HIPAA regulations.
- Facilitate discharge planning by collaborating with healthcare teams to ensure smooth transitions from acute care settings.
- Provide education and support to patients and families regarding treatment options and resources available.
- Monitor patient progress and adjust care plans as necessary based on ongoing assessments.
- Maintain accurate documentation of case management activities and patient interactions.
