Compassionate Hospice Clinical Lead

Hospice Clinical Manager | Registered Nurse (RN) or Social Worker (MSW)

We are seeking a compassionate RN or MSW with a strong passion for hospice care and leadership to oversee one of our Hospice Community Teams.

Main Responsibilities

• Team Oversight: Supervise and lead an interdisciplinary team of professionals, ensuring high-quality patient care 24/7.
• Care Planning: Direct and participate in weekly interdisciplinary group meetings to discuss patient progress and develop care plans.
• Quality Assurance: Supervise daily activities and ensure quality patient care is provided on a continuous basis.
• Manager-on-Call Rotation: Participate in the manager-on-call rotation with other Hospice Managers to provide ongoing support.

Requirements

• Licensure as an RN or MSW in the State of Michigan.
• At least 1-2 years of management experience in a skilled nursing or med/surg setting.
• Prior hospice nursing or social work experience is highly preferred.
• Strong computer skills to chart in the EMR are essential.
