Urgently hiring reliable LPN for home care client.
68-year-old individual with a history of moderate intellectual disability, seizure disorder, osteoarthritis, and a recent stroke with residual physical limitations.
- Assist with activities of daily living (ADLs), including bathing, dressing, toileting, and hygiene.
- Provide hands-on support for transfers and mobility. Margaret uses a walker at home and a wheelchair in the community.
- Monitor for seizure activity, particularly at night, and ensure safety post-seizure.
- Assist with feeding cues to prevent overstuffing and ensure safe swallowing.
- Provide reminders for hydration and dietary needs (gluten-free).
- Be present or on-site at all times due to fall risk and incontinence needs.

- Perform PRN straight catheterization.

- Support medication administration and monitoring for side effects .
