Job Overview
We are seeking a compassionate and dedicated Registered Nurse (RN) for our Home Health division. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong clinical background, excellent communication skills, and the ability to work independently while managing various patient needs.
Key Responsibilities:
- Client Meetings: Meet with 3 clients per month, once virtually and once in person, to assess their health and wellbeing.
- Documentation: Complete the RN oversight form in the Electronic Medical Record (EMR) system after each visit.
- Communication: Report any issues or concerns regarding the client’s care to the Director of Nursing (DON) promptly.
Qualifications:
- Active Registered Nurse (RN) license.
- Experience case management and client assessments
- Strong interpersonal and communication skills.
- Experience with Electronic Medical Records (EMR) is preferred.
Compensation:
- Pay rate of $40-45 per visit. (Based on experience)
