FaithCare Hawaii is seeking compassionate and reliable caregivers to join our team and provide exceptional support to our clients in the following areas;
Honolulu
Note : Applicants must reside in Oahu, Hawaii, or be willing to relocate if coming from the US mainland and have the authorization to work in the United States.
DUTIES
Provides personal care and related services in the home under the direction, instruction and supervision of the Supervising Nurse :
Personal hygiene and grooming;
Bathing, skin care, oral hygiene, hair care, dressing, assistance in toileting;
Assistance with ambulation, mobility, transfers, and positioning; and
Personal care services do not include the management of the client's money or finances, or the balancing of the client's checkbook.
Tasks to be performed by a PCA must be assigned by and performed under the supervision of the Supervising Nurse who will be responsible for the client care provided by the PCA.
Under no circumstances may a PCA be assigned to receive or reduce any intravenous procedures, or any other sterile or invasive procedures, including enemas.
RESPONSIBILITIES
Follows the service plan to provide, safe, competent care to the client.
Helps the client to maintain good personal hygiene and assists in maintaining a healthful, safe environment.
Plans and prepares nutritious meals when instructed to do so by the service plan
Assists the client with ambulation
Encourages the client to become as independent as possible according to the service plan.
Attempts to promote client's mental alertness through involvement in activities of interest.
Provides simple emotional and psychological support to the client and other members of the household and establishes a relationship with the client and family that transmits trust and confidentiality.
Report any change in the client's mental or physical condition or in the home situation to the supervisor.
Performs routine housekeeping tasks as related to a safe and comfortable environment for the client, as instructed by the Service plan :
Routine housecleaning such as sweeping, mopping, dusting, making beds, washing dishes, cleaning toilets, sinks, showers, or bathtubs, or disposing of rubbish;
Care of clothing and linens, including washing, drying, ironing, and mending;
Shopping for household supplies, clothing, and personal essentials;
Running errands or picking up medication;
Shopping for food or preparing meals;
Escorting the client to medical care services or to nutritional or recreational programs and
Assisting with simple health care routines such as reminders to take oral medication, to maintain diet restrictions, or to perform recommended exercises.
Prepares a visit note in a timely manner and submits it within the caregiver portal for documentation.
Confirms the scheduling of visits on a weekly basis.
Works with personnel of other community agencies involved in the client's care as directed by the supervisor.
Attends in-service as required by state regulation.
JOB CONDITIONS
The ability to drive and to access clients’ homes, which may not be routinely wheelchair accessible, is required.
Hearing, eyesight, and physical dexterity must be sufficient to report on the client's condition, to identify a change in the client's condition, and to perform client care or assigned tasks.
On occasion, may be required to bend, stoop, reach, and move client weight up to 250 pounds; lift and / or carry up to 30 pounds.
Must be able to communicate clearly, both verbally and in writing, in English.
Must be responsible for providing reliable transportation to get to the client's home (driving, commuting, personal drop off, etc.).
COMPANY INFORMATION
Has access to all client medical records, which may be discussed with the Registered Nurse and the Supervising Nurse.
QUALIFICATIONS
Must have a high school diploma or equivalent.
Successful completion of a basic nurse aid training program approved by the Department OR has successfully completed coursework that qualifies a person as a certified or licensed health care professional.
Must be free from health problems that may be injurious to client, self, and co-workers and must present appropriate evidence to substantiate this. The agency may require a current physical examination signed by your primary care provider.
Must comprehend the basics of personal care, housekeeping, and meal preparation.
Must understand and respect clients, including ethics and confidentiality of care.
Must have a complete criminal background check.
Must have current FA / CPR certification.
Must have current TB Clearance (2-step).
Must have other documentary requirements (ID or passport, SSN card, etc).
