Social Services Coordinator - Full Time /Exempt
Falcons Landing is seeking a highly motivated, energetic and talented Social Services Coordinator to serve our wonderful population of military retirement heroes. Responsibilities include provision of social services from the time of admission, care coordination, and discharge planning for residents of our skilled Nursing facility, The Johnson Center.
Located west of Washington, DC in Loudoun County, Northern Virginia, Falcons Landing is a premier Life Plan Retirement Community. As a privately held not-for-profit organization, we are committed to providing a supportive and caring work environment. Falcons Landing features a blend of residential homes, spacious apartments, and three top-tier healthcare residences.
Key Responsibilities:
- Provide social services and support for residents, ensuring quality care and satisfaction throughout their stay.
- Manage difficult and sensitive situations, demonstrating empathy and professionalism.
- Coordinate care and discharge planning, liaising with external agencies and ensuring smooth transitions.
- Maintain and manage confidential and sensitive information with discretion.
- Work independently while demonstrating strong interpersonal skills to effectively collaborate with residents, families, and the care team.
Qualifications:
- BSW (Bachelor of Social Work) or a degree in a related human services field required.
- Minimum 2 years of experience as a social worker in Long-Term Care or a similar setting.
- Strong experience with discharge planning, managing referrals, and coordinating services.
- MDS knowledge (Minimum Data Set) required.
- Excellent verbal and written communication skills, with a focus on clarity and professionalism.
- Ability to manage sensitive situations independently with strong problem-solving abilities.
Salary:
