License : Valid Physical Therapist License

Must-Have : Ability to provide therapy programs for patients of all ages
Job Description :
- Support the treatment of illness and / or injury through the use of physical exercises and therapies.
- Provide therapeutic care and teach rehabilitation skills to clients and / or caregivers.
- Implement preventive and rehabilitative physical therapy procedures individualized to the client.
- Develop and revise a plan of care based on assessments, client needs, history, diagnosis, and goals.
- Perform or monitor treatments using physical agents such as heat, water, and electricity.
- Follow utilization guidelines and protocols and document all necessary care thoroughly and accurately.
- Ensure compliance with Medicare, State, Federal, JCAHO, insurance, and payer requirements.
- Maintain client confidentiality and participate in quality management and required in-service programs.
- Conduct supervisory visits for physical therapy assistants and home health aides as applicable.
- Participate in staff meetings, case conferences, and customer service programs.
- Deliver high-level customer service to all internal and external customers.
- Demonstrate leadership through communication, relationship building, personal development, and teamwork.
- Holiday coverage may be required.
