Families First Community Center (FFCC) is a non-profit organization in Ellsworth, Maine, focused on breaking the cycle of homelessness for families with minor children. We provide transitional housing and comprehensive wrap-around services for homeless families and those at risk of homelessness in Hancock County. Our goal is to equip families with the tools they need to achieve self-sufficiency.
We are seeking a Case Manager/Life Plan and Outreach Specialist with an MHRT-C, who has experience in case management and a passion for working with homeless families. The ideal candidate will be able to work independently, communicate needs effectively, and thrive in a team environment.
Position Overview: This role combines responsibilities under Section 13 (Targeted Case Management) and Section 17 (Community Support Services):
- Section 13 focuses on connecting clients to services and helping them navigate systems like MaineCare, housing resources, or mental health services. It is more administrative and coordination-based.
- Section 17 provides a deeper level of hands-on support, helping clients with day-to-day management of their mental health and life skills. It involves ongoing assistance to help individuals maintain independence and manage daily challenges.
In addition to providing case management under these two sections, the Case Manager will be responsible for offering 3-4 hours per week of direct assistance to individuals in the community who are experiencing homelessness or are at risk but are not currently engaged in case management. This group may not seek case management due to unwillingness to receive a mental health diagnosis or because they only need acute, short-term support. The outreach work and community case management work will involve meeting people where they are, including homeless shelters, campsites, or vehicles.
Key Responsibilities:
- Provide a mix of Section 13 and Section 17 services to eligible clients.
- Outreach support for homeless individuals in the community who do not wish to engage in traditional case management.
- Meet individuals in need at locations such as shelters, camps, and vehicles to provide direct assistance and connect them to resources.
- Develop, maintain, and update Life Plans and treatment plans with residents and clients on a weekly basis.
- Ensure billable hours per week are submitted under MaineCare, with the potential for more.
- Navigate the Atrezzo system for MaineCare claims submissions.
- Build a client list by connecting with individuals and organizations in the community.
- Provide guidance and support to residents, including transportation to appointments and lining up services.
- Attend and participate in staff meetings, house meetings, and required supervision with the Executive Director and Clinical Supervisor.
Qualifications:
- Education: Minimum MHRT-C; Bachelor’s Degree in Social Work or related field preferred.
- Licenses and Experience:
- Valid Maine driver’s license.
- Experience with MaineCare or Maine Housing preferred.
Work Environment & Physical Demands:
- Work both in the office and out in the community, meeting clients at their residences, shelters, or public spaces.
- Physical activity may include sitting, standing, driving, and lifting up to 30 pounds.
