Under the direction of FFHC Health Care Specialist (LPN) & Sr Health
Care Specialist (RN)
Summary:
This STNA/Caregiver position aids in daily living and personal care services in assigned FFHC patients' homes in accordance with health assessment plan that is aligned with the approved physician-ordered service plan.
Work Hours:
This is a full-time or part-time position which requires various fixed and flexible shifts hours for weekdays and weekends as needed per Agency client's strength. MUST BE OPEN TO WEEKDAYS AND WEEKENDS
Duties and Responsibilities:
- Provide healthcare services for male resident in his home (Struthers, Ohio).
- Provide client with personal care including personal hygiene
- Provide incontinent care and assist client with bathroom needs
- Maintain accurate, legible patient documentation including date, time, task, care, and service to be provided to clients doctors and FFHC nurses daily.
- Maintain daily records of clients vitals to be provided to his doctors and FFHC nurses daily
- Maintain open communication between families and FFHC care staff (i.e., LPN, RN, STNA, Chief Nurse) regarding any medical and emotional conditions and concerns.
- Assist with housekeeping, meal feeding and/or prep
- Ensure highest client safety and well-being.
- Excellent written and communication skills. Ability to use EMR system/app with phone.
- Complete Performance Evaluation Plans, Annual Evaluations and Meet with Supervisors as directed
- Maintain confidentiality, integrity, quality care, and professionalism.
Qualifications:
- Ability to Pass Caregiver Certification and Skills Lab Training
- Up-to-date CPR/ First Aid Certification and Maintenance of Mandatory 75HRS of CEUs every 2 years.
- Must be physically and mentally fit to handle the demands of caregiving.

- Ability to turn resident as requested
- Ability to make bed while resident is in bed
- Must have working phone/email to communicate daily work
- Must be able to clock in/out from phone with location services enabled
