As the Community Health Director, you'll lead our Community Health department, develop strategies to improve health outcomes, and champion health equity through partnerships, advocacy, and creative solutions. You’ll work closely with teams, healthcare systems, and community organizations to make a real difference in the lives of those we serve.
What You’ll Do:
- Lead, inspire, and manage our Community Health department staff.
- Integrate health equity and social determinants of health across FCHC, working with teams to improve care delivery, health management, data collection, and advocacy.
- Build powerful community relationships with stakeholders across sectors to advance partnerships and improve health for our clients.
- Establish strong referral relationships with local agencies and organizations.
- Represent FCHC on community task forces and work with others who share our goals of enhancing community health.
- Be a strong advocate for our clients, helping guide policies and programs that ensure health equity.
- Work with teams to leverage data insights and understand the broader needs of FCHC clients, including care transitions, access to care, and care management.
- Use evidence-based practices to support individuals and communities in reaching their health goals.
- Provide public health services, including home visits, to clients in need.
- Take the lead on grant writing, helping secure funding that supports our mission and programs.
What You Need:
- Bachelor’s Degree in Social Work or related field required.
- Master’s Degree in Social Work or related field from an accredited school or related field required.
- 5 years of leadership experience managing a team in a healthcare or community-based organization focused on marginalized communities.
- Strong experience within a Community Health Center, Federally Qualified Health Center, or a complex community-based organization.
- Grant writing expertise, with a successful history of securing federal, state, or foundation funding.
- Ability to transform strategic goals into actionable plans and use data insights to improve health outcomes.
- Excellent communication skills, both written and verbal, with experience presenting to executive teams and boards.
- Ability to create and manage budgets and business plans.
