The Position
As a Case Manager, you will work to improve the quality of life for clients, caregivers, and their families. Through consultation and personalized assessment, you will assist the team in creating tailored care plans to help the client achieve personal goals and, in doing so, help release the stress and strain family caregivers often face. We intuitively prepare for a range of care challenges. Essentially, we are a lifeline – a place our clients turn to for short or long-term support and a resource that connects them to the best care options. Qualifications.
Qualified applicants for this Case Management position will have:
- A college degree or the equivalent in education and experience
- Skill in coordinating and supervising the work of others
- Excellent customer service skills
- Good communication skills - both verbal and written
- Ability to self-manage while ensuring timely and high-quality work
- A strong passion to work with the population that we care for
- Salary range $34,000-$36,000 depending on experience
