Family Centers Health Care seeks a bilingual, energetic, and compassionate individual with referral management and community outreach experience to join the team at Family Centers, a non-profit organization offering health care, education and human services to children, adults, and families living in Lower Fairfield County. The health center provides primary healthcare, dental and mental health services to those living in our communities, public housing, and the surrounding neighborhoods.
The Role
The Patient Navigator serves as a liaison between patients, healthcare providers and community partners. As part of the medical care team, the services provided by the Patient Navigator improve patient health outcomes and assist patients in managing their own health care and becoming independent, informed consumers of health care services. Primary responsibilities include medical referral management, assistance with accessing concrete services and linkage to outside services. This position requires strong communication skills, empathy, and a deep commitment to patient satisfaction.

Responsibilities Include
- Coordination of Care: Support patients with scheduling, referrals, and specialty care follow-up, ensuring clear communication and guidance.
- Specialty Provider Liaison: Collaborate with external providers to schedule appointments, share medical records, and obtain patient care updates.
- Referral Processing and Education: Process and track referrals, ensuring accuracy and compliance with clinic policies and insurance. Educate patients on the referral process and necessary preparations.
- Social Services Referrals: Make timely referrals to address patients’ social health needs, assist with documentation to improve service access.
- Comprehensive Needs Assessment: Use screening tools to identify social health needs and barriers to care, helping patients overcome challenges to health access.
- Address Barriers to Access: Identify and provide solutions to barriers like transportation, financial issues, or language challenges to ensure access to care.
- Continuity of Care: Ensure proper documentation and coordination for specialist visits, supporting smooth care transitions.
- Communication and Follow-up: Act as the main contact for patients, providing updates and addressing concerns about referral status and appointments.
- Collaboration with Care Teams: Coordinate with healthcare providers to ensure smooth referral and care processes across specialties.
- Documentation and Reporting: Maintain accurate referral records, generate reports on referral metrics, and support decision-making.
- Compliance and Standards: Ensure adherence to HIPAA, insurance, and clinic policies in referral activities.
- Other Duties: Assist with surveys, maintain professionalism, and perform additional tasks as assigned by the Medical Operations Manager.
- Bi-lingual Spanish required
- Associate’s degree in human services, health services, medical technician or related degree preferred
- Knowledge of healthcare systems, medical terminology, insurance requirements and referral protocols
- Proficient in general computers skills to include Microsoft Office Suite, Outlook, and digital communication platforms, typing, data entry and management
- Familiarity with Electronic Health Records Systems preferred
- Excellent communication and interpersonal skills.
- Strong organizational, problem solving and professionalism skills
- High level of cultural competency, emotional intelligence and teamwork
- Adaptability and ability to work autonomously with minimal supervision
