Social Worker for Lead Care Manager position (Desired start date: Immediate)
Position Overview
The Lead Care Manager (LCM) coordinates and implements organization-wide Enhanced Care Management. In this capacity, the LCM oversees and implements the provision of Enhanced Care Management (ECM) services for plan members under this LCM’s care.

The position will allow you to work with the best in your field healthcare professionals who have worked in our industry for many years by providing case management and advocating for their clients.
Desired Qualifications:
· Master's or Bachelor's degree in a Social Work field related to work with seniors, disabled adults, minors and youth.
· Experience working with minors and youth population of focus.
· Microsoft Office product expertise required (Excel, Word, PowerPoint, Outlook)
· Bilingual
· Strong interpersonal and communication skills, with the ability to connect with diverse populations.
· Empathy, cultural sensitivity, and a passion for improving community health and well-being.
· Excellent organizational skills and attention to detail.
· Ability to work independently and collaboratively within a team.
· Valid California Driver's license.
Job Types: Full-time, Part-time
Pay: $46,000.00 - $72,000.00 per year
Expected hours: 20 – 40 per week
