What are the responsibilities and job description for the Service Coordinator position at Family Community Services?
At Family Community Services, we are seeking a highly skilled Service Coordinator to join our team. The ideal candidate will be responsible for providing developmental screenings to children from birth to three years old and coordinating early intervention services if the child is determined eligible.
The successful Service Coordinator will establish trusting relationships with families and children, as well as service provider agencies and organizations involved in their care. They will implement service coordination and supportive services according to agency policies and procedures, including assessments of child and family needs, provision of information on procedural safeguards, and coordination of evaluations.
- Manages an active caseload of families and children participating in EI, spending approximately 50% of time providing direct service to families and children;
- Establishes trusting relationships with families, children, and service provider agencies;
- Coordinates early intervention services, develops individualized family service plans, and facilitates transitions to family-selected service options;
- Conducts developmental screenings and utilizes other assessment tools per training requirements;
- Ensures timely completion of IFSPs and maintains accurate records.
